To proste. Zainstaluj aplikację i po uruchomieniu zaloguj się na swoje konto (e-mail i hasło).

Następna synchronizacja pobierze wszystkie Twoje dane z sieci.

Subskrypcja pochodzi z Google Play, więc na nowym telefonie również musisz być zalogowany na właściwe konto.
